Membership terms
This is a direct dental membership program, not dental insurance. Membership requires a 12-month commitment. Monthly payments continue for the full 12-month commitment; the annual option charges $948 upfront and automatically renews at $948 every 12 months until canceled. Benefits begin immediately once enrollment and payment are complete. Membership must remain active and current to use benefits.
Except where required by law, payments and included services are nonrefundable and nontransferable, have no cash value, and do not roll over. The 20% member savings applies only to eligible dental treatment at Calabasas Dental Spa and cannot be combined with dental insurance benefits, other discounts, or promotional offers. All care is subject to clinical need and appointment availability.
Canceling annual renewal: Email info@CalabasasDentalSpa.com before your next annual renewal charge to stop the next renewal. Include your name and the email used to enroll; do not send card details. Your membership continues through the end of the paid 12-month period. Canceling renewal does not automatically refund the current annual payment; refunds required by law still apply.
